A sterile material is only sterile until something breaks the claim: a torn pouch, a passed date, a box stored in the sun. The check that catches all of it takes about thirty seconds a unit and belongs at the start of every setup, not at the end of a bad week. Here it is, in the order that fails most often.

What to do with a fail
A failed unit does not go in the bin straight away. It goes in a bag with a note of the lot and a photo of the problem, and the supplier hears about it the same day. That is how a single bad pouch becomes a known problem with a lot instead of a story nobody can check. Single use means single use
Cartridges, grips, film, gloves: once a sterile unit has been opened it is used for one procedure on one client and then disposed of. There is no cleaning step that restores the claim on the box. Re-sterilising a single-use item is not a saving; it is a liability with your name on it.
Storage is part of the check
Most failures we hear about are not manufacturing defects; they are boxes kept on a windowsill or under a sink. Keep sterile stock in a closed cupboard, boxes upright, oldest lot at the front. When a new restock arrives, put it behind the old one, not in front of it.
